Please fill out this form if you are interested in scheduling an appointment with me. Please have your deposit ready to go so that we may book you promptly when I contact you.
DEPOSITS (25%): A Non-Refundable deposit of $200 - $500, is required after we discuss your request and decide on a date/time. If you do not send the deposit promptly I will not be able to save the date/time for you. Deposits are only transferable ONCE. Meaning if you must reschedule your appointment you can only do so once before losing your deposit. If this happens you must put down ANOTHER deposit to reschedule again or book.
